Caring for people of all ages since 1991, Capital Medical Group’s Family Medicine Division offers resources on the east and west sides of Frankfort as well as in Lawrenceburg. Our goal is to help you prevent illness and maintain your best possible health. We can help identify potential health problems and treat many of your current and chronic medical issues. Our training is second to none, our facilities comprehensive and convenient, and we’re available when you need us.
We know you can’t always take off on weekdays, so we feature evening and weekend hours, plus walk-in options.
Wellness means more than an annual checkup and a flu shot. It means having a positive attitude, a healthy lifestyle, and guidance from a team of medical professionals who know you and can assist when necessary.

New patients are always welcome. Please call the office to schedule an appointment.
For your first office visit, arrive 15 minutes before your appointment to fill out new patient registration forms. For your convenience, these are provided here to complete and bring with you.
Please print these forms, fill them out, and bring them to your first appointment. (We accept most major insurance carriers.)
Physicals and well-child visits should be scheduled in advance to ensure you will be seen by your preferred provider and to make sure appropriate blood work is done.
Sick visits are scheduled the same day whenever possible. We advise you to call early for an appointment. Walk-in patients are seen on a first-come, first-served basis AFTER scheduled appointments and emergencies.
Make an appointment for each person to be seen. We need to allow adequate time for each patient. Please do not ask your doctor to see additional patients who do not have an appointment.
Please arrive on time and if you need to cancel, call the office as soon as possible. Appointment no-shows may result in discharge from the practice.
Bring any forms that your provider must complete as well as the following:
1) Your insurance card
2) Photo ID
If you have an emergency, please go to the nearest emergency room or call 911.
If you have an urgent problem after hours, please call the office and you will be given the on-call provider’s number.
When calling to leave a message, please allow reasonable time for your provider to return your call.
We require 24 hours’ notice to refill prescriptions. Please call at least two days prior to running out of your medication, in case there is a problem with your refill.
We accept cash, check, or credit cards (Visa, MasterCard, American Express, and Discover).
Self-pay patients are required to make a $56.00 deposit prior to being seen. After your visit you will be billed for the remainder of the charge.
Insurance patients must pay co-pays at time of service and have their insurance card available.
If you have any questions regarding your bill or insurance, please contact the billing office at (502) 223-5811, 8:00 a.m. – 4:30 p.m., Monday – Friday.
